I am writing to advise you that following a search of our paper and electronic records, I have established that the information you requested is not held by the NHS Business Services Authority.
We do not collect or hold information as to why someone was issued a PCN. The sampling/checking process simply checks for the declared exemption against a randomly selected prescription and where we can confirm a valid exemption the case is closed and those not confirmed will be issued a PCN.
