Request: Dear Sir/Madam. This is a request for information under the Freedom of Information Act 2000.My request relates to prescriptions handed at out by NHS doctors via online consultations; specifically, for patients registered with the "GP at Hand" service provided by Babylon (https://www.gpathand.nhs.uk/switch-now?utm_source=google&utm_medium=cpc&utm_campaign=1779458318&utm_adgroup=70826532682&utm_keyword=%2Bgp%20%2Bregistration&utm_ad=343470650154&utm_location=1006886&gclid=Cj0KCQjwr-_tBRCMARIsAN413WTPDdMv-gMiKk6Yz76P9HdE3paUvP9gyjgi74n8BpNRgKTagKN7Y9waApokEALw_wcB). I would like to request the following information. Please break down your response by NHS region. I am interested in data from the last 6 months up until October 2019. 1) "Babylon GP at Hand" sometimes prescribes medicines through an online consultation service, without a GP ever seeing the patient in person. How many prescriptions for any kind of medication were electronically sent to patients in this way that I have just described?  2) How many of those prescriptions were later found to be in error?3) How many patients that were given a prescription by a doctor following a consultation with "GP at Hand" made a complaint of misdiagnosis, or made a follow up appointment with the service within 2 weeks and received different diagnosis for the same issue originally reported?4) Please could I have any documents or internal communications relating to any NHS internal reviews of the effectiveness of the "GP at Hand" programme''s prescription services.Please supply the information in an electronic format.  If you feel that a substantive response to this request is not possible within a reasonable time frame, I would be grateful if you could contact me and provide assistance as to how I could refine that request. Furthermore, if part of the information in the scope of this request catches one or more exceptions please remember to apply a presumption in favour of disclosure, as stipulated by regulation 12(2).  I look forward to receiving your response within 20 days.
