Request: How many other patients claiming Universal Credit at my dispensary have had their applications rejected and fines imposed because they earn too much and how many of these penalty notices have subsequently been appealed against and the penalty disregarded [On 7 February 2018 you clarified the following:] As the current form does not have a space to claim an exemption based on universal credit I was advised in good faith to tick box M . Because the form does not have this dedicated section for Universal Credit with a note that there is a maximum permitted earnings i have now received a penalty notice. Within the penalty adjudication process it must be possible to establish the breakdown of (a) the reasons why a claim was judged not to comply with the regulations under K and M e.g Job Seekers or Universal Credit and (b) the reasons why a claimants appeal was upheld and the PCN cancelled. Please provide this information. In addition Please supply the number of overall Penalty Charge Notices (PCNs) issued to patients at my pharmacy for Sections K and M and how many have had their PCNs cancelled .
